Take Action Today: Submit Documentation for Immunization or Medical / Religious Exemptions

Dear Faculty and Staff Colleagues:

The electronic form allowing professors and staff to submit COVID-19 vaccination and exemption documents is now available on the MySlice portal. This can be done on a laptop or desktop computer with the best results using Chrome or Firefox browsers. Mobile devices are not supported at this time.

As announced last week, Syracuse University will require all faculty and staff to submit documented proof of immunization status or information to support their request for medical or religious exemption. This process will be done electronically and the documentation you provide will be kept in a secure and confidential electronic file, separate from your official employment records.

We ask that you submit your documentation as soon as possible in order to allow sufficient time for it to be approved before the start of the spring semester (January 18, 2022). This message details the processes for submitting documentation for review and verification by the University.

Submit proof of vaccination:

To submit proof of vaccination, including booster vaccination, please follow this process. You must answer all questions, click all the checkboxes and upload your proof of vaccination for your form to be approved. The form will not be saved until you click on the “Submit” button.

  • Log in to MySlice. Note that as of December 12, the icons of MySlice have changed, but the functionality remains the same.
  • On the “Employee Home” page, select “Employee Resources”.
  • In the “Resources for Employees” section, select “Electronic Forms”.
  • Once in “eForms”, click on the triangle next to “Human Resources”.
  • Under “Human Resources”, select “COVID-19 Immunization Status”.
  • In the “COVID-19 vaccination status” form, enter the exact dates of your vaccinations as well as the type of vaccination received. (Please do not enter future dates even if you have a scheduled appointment.)
  • Upload proof of vaccination by clicking on the option “Attach proof of vaccination”. Acceptable forms of proof include:
    • A photo of your COVID-19 vaccination record clearly indicating the vaccination dates
    • Immunization record from a public health or state immunization system, such as the Excelsior pass
    • Immunization record from a health care provider or pharmacy
    • Medical records or other official documents verifying vaccination status with information on the name of the vaccine, dates of vaccination and the name of the healthcare professional or clinic site that administered the vaccine
  • Once you have completed the step above, enter your preferred phone number or email address so that Human Resources can contact you if there are any questions regarding your submission.
  • Check the box beside to certify that the information provided is correct and complete.
  • Click the “Submit” button at the bottom of the documentation to submit your form.
  • You will receive an email on your Syracuse University account once your information has been reviewed and verified. Due to the high volume, it may take several business days.

Submit a medical exemption:

To request a medical waiver, please follow these steps. You must answer all questions, click all the checkboxes and download the Supplier certification [PDF] for your form to be approved. The form will not be saved until you click on the “Submit” button.

  • Download and complete the upper part of the “Health Care Provider Certification of Medical Exemption from the COVID-19 Vaccination Requirement for League Employees ”(“ Supplier Certification [PDF]“).
  • Ask your health care provider to complete the “Supplier certification [PDF]. “
  • Once done, log into MySlice. Note that as of December 12, the icons of MySlice have changed, but the functionality remains the same.
  • On the “Employee Home” page, select “Employee Resources”.
  • In the “Resources for Employees” section, select “Electronic Forms”.
  • Once in “eForms”, click on the triangle next to “Human Resources”.
  • Under “Human Resources”, select “COVID-19 Immunization Status”.
  • In the “COVID-19 vaccination status” form, click on “Request a medical exemption”.
  • Check the boxes to attest to the certification of your answers.
  • Download your finished Supplier certification [PDF].
  • Once you have completed the step above, enter your preferred phone number and email address so that someone can contact you if there are any questions regarding your submission.
  • Click the “Submit” button at the bottom of the application, which will automatically send your completed application to the United States Disability Act (ADA) Coordinator at Syracuse University or their delegates who will review and hire you. in an interactive process, if necessary.
  • If your request is approved, the Syracuse University ADA Coordinator or their representatives will record your vaccination status as meeting COVID-19 vaccination requirements.
  • If the exemption is for a limited period of time, you will need to comply with COVID-19 vaccination requirements at the end of that period.
  • Please note that the review of your exemption request may take up to five to 10 business days and this review deadline will not extend any time frame to comply with COVID-19 vaccination requirements.

Submit a religious exemption:

To request a religious exemption, please follow these steps. You must answer all questions and click all the checkboxes for your form to be approved. The form will not be saved until you click on the “Submit” button.

  • Log in to MySlice. Note that as of December 12, the icons of MySlice have changed, but the functionality remains the same.
  • On the “Employee Home” page, select “Employee Resources”.
  • In the “Resources for Employees” section, select “Electronic Forms”.
  • Once in “eForms”, click on the triangle next to “Human Resources”.
  • Under “Human Resources”, select “COVID-19 Immunization Status”.
  • Once here, click on “Request a religious exemption”.
  • Check the boxes to attest to the certification of your answers.
  • Then upload a Word document or enter your information in the electronic form along with your personal answers to the following questions:
    • What religious principles do you sincerely defend that are against receiving any of the COVID-19 vaccines? Please note that social, philosophical, political or economic views and personal preferences are not considered religious beliefs.
    • Do your sincere religious beliefs prevent you from receiving other vaccines or taking certain medications? If not, describe the religious basis that specifically prevents you from receiving any of the COVID-19 vaccines.
  • Enter your preferred phone number or email address in case you have any questions regarding your submission.
  • Click the “Send” button at the bottom of the application.
  • If your request is approved, you will be recorded as meeting COVID-19 vaccination requirements.
  • Please note that it may take up to five to 10 business days to review your application and this review deadline will not extend any time frame to comply with COVID-19 vaccination requirements.
